Ready to Start Consultation Service?

Step 1

Individual completes assessment with Lead Agency.

Step 2

Lead Agency or Case Manger authorizes a service agreement for Consultation Services.

Step 3

Participant or Case Manager completes a referral form for Consultation Services.

Eligibility

Eligibility for Consultation Services

Completed Assessment

A person interested in accessing Consultation services must receive an assessment to determine their need for services.

Live in their own home

A person receiving services must live in their own home, and cannot live in a hospital nursing facility, ICF/DD, Foster care setting licensed for more than six people or housing owned by Orion ISO.

Enrolled in MHC Program

A person receiving consulation services must be enrolled in one of Minnesota’s health care programs

Direct own care

Be able to direct their own care or have a representative who can act on their behalf.

Educate

We will help you understand CFSS Program with personalized educational sessions. We’ll walk you through the eligibility criteria, service options, and how to best leverage the program to meet your goals.

Write

Our consultants will work closely with you to develop a customized plan that aligns with your participant’s needs and preferences, ensuring clarity and compliance with program guidelines.

Review

Reviewing and Submitting CFSS Service Plans for Approval: Our goal is to ensure accuracy and completeness before submission, saving you time and ensuring smooth approval processes

Support

We will provide ongoing support, responding to questions and concerns that come up throughout plan year, assisting you make changes, if needed.

Starting in October, 2024 Minnesota Department of Human Services (MN DHS) will require Community First Services and Supports (CFSS) participants to work with a Consultation Service Provider to help support you in writing, reviewing, and approving your CFSS Budget Model support plan.
